What is the reason for the loss of the mechanical seal screw in an operating oil pump?

When it stopped, I found a bunch of screws missing. Is that okay? It has been running for almost a month
The tightening force of the threaded fit is insufficient, and the threads of the gland or the screws are deformed or have abnormal dimensions.
That is the screw for the sealed locking sleeve; if it falls off, it’s not a big problem – there’s a ring of screws.
As can be seen from the diagram, the shaft sleeve positioning screw has not fallen off, while the mechanical seal positioning piece has fallen off; this does not affect operation.
Before installation, the three positioning tabs of the cartridge mechanical seal hold the shaft sleeve in place; these tabs determine the amount of spring compression. After the cartridge mechanical seal is pressed against the pump, the set screws used to tighten the shaft sleeve are tightened, and the mechanical seal is then positioned. Finally, release those three cards so that they move away from the bushing, and secure them in a position that does not affect the rotation of the bushing. So even if it falls off, it doesn’t affect operation. The set screw that holds the shaft sleeve in place must not fall off; if it does, the moving ring of the mechanical seal will move, resulting in leakage from the mechanical seal.
