Dear friends who have taken the pressure vessel auditor exam, what exactly are considered major errors in inspection reports?
Although I am not an inspector, in my opinion the most critical issues in drawing review are incorrect assessments of equipment strength and opening sizes; followed by the level of hazard posed by the media, including the proper classification of such media, as well as incorrect material selection. This also includes various requirements regarding material testing and re-inspection – please add anything that was missed below.
