Issue with the outlet temperature of the coolant water in the cooler

I read in the design manual that the outlet temperature of the coolant water in a cooler generally does not exceed the outlet temperature of the medium being cooled. Why is that? Can the medium outlet temperature be set to equal the cooling water inlet temperature? If that is the case, what should be done if the smaller value of Δθ in the calculation of the average temperature difference for the heat exchanger is 0?
It’s simply impossible to achieve from a theoretical calculation standpoint.
