How to implement tag-out and lock-out when performing pipeline maintenance or working in confined spaces? Especially when there is concurrent work by multiple parties, how should the tag-out and lock-out procedure be carried out? I would appreciate some guidance from experienced seniors.
For the entrances and exits to confined spaces, we have not yet fully implemented labeling and locking procedures for major overhauls. The requirements set by our factory are as follows: when working in confined spaces, safety warning signs must be placed outside those spaces. During the period when work is suspended (including when work is halted), a sign reading “DANGER! Use “No Entry” signs or other sealing measures to prevent unauthorized access.
During pipeline maintenance or work in confined spaces, as well as in situations with concurrent operations. At the early stage of the task, all parties involved in work related to confined spaces should be contacted to conduct a JSA analysis, and the identified hazards—such as electrical elements and process media—should be isolated; it is necessary to determine whether isolation is possible (by using blind plates). Make proper records, and the keys to the locks used for isolation should be collected and placed in a common lock box. During the operation, confined spaces should be inspected at regular intervals; it is important to have supervisors on site who are well-versed in the emergency response plans, and the workers themselves must also wear appropriate safety gear. Finally, at the end of the work, the site is cleaned and inspected to ensure that no tools or equipment used in the work remain inside the confined space.
If it’s already labeled, then there’s no need to lock it, right? Do we still have to lock the label? You can ask domestic lock manufacturers; they should have knowledge in these areas

The listing and locking should be explained separately, right? In confined spaces, signs must be placed when people are inside; once the work is done, locks are used only if conditions permit to prevent others from entering accidentally. There seems to be a misunderstanding here
It’s a factory that has been in operation for over fifty years, and all aspects of its operations are quite well-developed. Some kind of system involving signs and locks was introduced in our factory over a year ago; I feel that this system is more of a formality than anything meaningful. The frontline employees and managers are quite resistant to it, and at present the factory is turning a blind eye to it. I think it will probably be abolished eventually.

I think it’s almost done. Our chain is made of plastic; I suggest replacing it with steel. The lock mechanism is ordinary, and I recommend using a more complex one. However, I still suggest conducting background checks on all personnel – no amount of protective measures can prevent malicious individuals from causing damage. ;P
