This led to the release of large amounts of toxic gases after the fire broke out, resulting in numerous casualties ； ——The particularly serious accident of the Sinopec Donghuang oil pipeline leakage and explosion on November 22, 2013, in Qingdao, Shandong, was also caused by unreasonable planning and design. The area where the accident occurred had chaotic planning, with the oil and gas pipelines being too close to surrounding buildings; in particular, the engineering design for the intersection between the oil pipeline and the underground channels was inadequate, which ultimately led to this tragedy. There are too many such examples. I remember a thermal power plant in Shandong where corrugated hoses were used in the design of the high-pressure lubricating oil pipelines for the turbines. Lack of design experience, inadequate design review, and lowering design standards to save costs are common problems.

On April 28, the Office of the State Council Safety Committee issued the \"Guidelines for Addressing the Root Causes and Preventing Severe Accidents,\" from which I quote the following: \"(1) Strictly regulate access through planning.\" Explore the establishment of a special safety planning system, incorporate safety planning into local economic, social, and urban development plans, and strengthen coordination and integration among these plans. Strengthen the preliminary analysis of safety risks in urban and rural planning, improve the safety standards for planning, design, and construction in these areas, develop a system for assessing safety risks associated with investment promotion, enforce strict safety reviews for the construction of high-risk projects, conduct scientific evaluations regarding the location and layout of high-risk enterprises, and strictly prohibit the establishment of densely populated areas around such projects in violation of relevant national and industry standards. I just want to say, why did it take so many years to realize this problem! On August 5, 1993, wasn’t the sound from Qingshui River in Shenzhen faint? Isn’t the loss significant? Isn’t the lesson profound enough? If the flaws in this system design had been fixed right after the accident back then, would there still have been such a hell on Earth as 8.12? !
