Vibration in the gas cooler return pipeline

We have a hydrogen cooler; the return pipeline is vibrating violently. I could hear knocking sounds coming from inside the heat exchanger. The temperatures at the inlet and outlet of the fluid are around 100 degrees and 35 degrees respectively. What could be the reason for this?
As a supplementary note: Our factory experienced an instance of freezing damage in the past; the valve flanges on the coolers were damaged due to freezing. Since there was no external leakage, it wasn’t given any attention. The pressure of hydrogen is 16 kilograms, and the pressure of circulating water is 3.5 kilograms
Suspected internal leakage. If it’s possible to stop and conduct a check, do so; otherwise, open the cooling water condensate valve and use a hydrogen detector for inspection.
