(III) Sequence of events surrounding the accident

July 25, 23:00 | Catalyst was being loaded into Reactor No. 3 in the maleic anhydride production facility (hereafter referred to as Reactor No. 3). Since the catalyst needed to remain dry, instrument air had to be supplied to the reactor. Workers from Jiangning Company, namely Jiang and Chang, opened the valves for the instrument air pipelines connected to the reactor.
