Analysis of the reasons for reverse rotation of the stop valve

I was working this afternoon, setting up a liquid level control for a water pipe. The water supply valve is a manual gate valve; the pressure before the valve is 1.9 megapascals. I turned it two or three notches to allow water to flow through, and suddenly the valve opened on its own due to the force of the water.
It’s possible that the copper sleeve of the valve is damaged and no longer has threads.
I’m also waiting for an expert to answer. . . .
It’s so smooth now; will the filler leak?
It is likely that the threads of the valve stem and the valve disc have come loose; the valve stem is unable to hold the valve disc in place. Can you listen to see if there is any noise coming from the valve?
At first, there was a lot of noise inside the valve, and the vibration was quite severe. If the valve disc falls off, I’m not quite sure why that happens. The operation of the valve is driven by its stem; then why does the stem rotate under the pressure difference between the front and back? Could it be due to excessive vibration?
At first, the packing didn’t leak. After adding water and closing it, it started leaking. Tightening the packing stopped the leakage.
It seems the packing is damaged; if that’s the case, it needs to be replaced
