The welding assistants did not wear safety helmets as required, and should not rest too close to live electrical devices. The oxygen cylinders and acetylene cylinders were not stored separately, and the acetylene cylinder was leaking

The distance between the two bottles is insufficient from the ignition point ； 3. Acetylene cylinder leakage ； 4. The distance between the two cylinders (combustible gas storage tanks) and the distribution panel is insufficient ； 5. Two bottles spilled and not secured ； 6. The guardian was not wearing safety equipment (helmet, gloves, mask, goggles) ； 7. Guardian (on duty) leaves post ； 8. The person performing hot work did not wear all the necessary safety protective equipment (welder’s gloves, goggles, dust masks, etc.) ； 9. Tie the oxygen-acetylene strap ； 10.
