Equipment Accident Management System

I would like to ask all of you here: what are the management systems in place for dealing with equipment accidents? The accident management system currently in use in our organization covers very little regarding equipment issues, and it is not practical either; only when a serious accident occurs is it that the person in charge of each workshop is held accountable. I believe this approach is incorrect. I think that under the new equipment management system, once an accident occurs, the person in charge, the primary responsible party, should be held accountable first. Which sea friend could take inspiration from this system? Thank you!
You can refer to the provisions on accident handling in the Work Safety Law. Behind any accident lies a management issue; if one digs deep enough, there is no one in charge of the workshop who cannot be held accountable. Each factory has its own circumstances, and regulations must first be practical and enforceable.
