Opening reinforcement

Dear experts, when performing calculations for strengthening holes created due to internal pressure, if the outer diameter of the pipe fitting is larger than the diameter of the hole, how is welding done in such a case?
The opening diameter is merely a term defined during calculation; it is not the actual size of the opening. What’s wrong with that?

What is the relationship between the sign of the calculation and the diagram you drew? What about the corrosion allowance? Should you include that in your diagram as well?
Same question: Is reinforcement always required when opening manholes? But I saw that some of the manholes on the atmospheric pressure tanks inside the unit were not reinforced. There are also things like gas tanks, whose inlet pipelines are quite thick; do they also require reinforcement? Are there any relevant standards or specifications for this?
When drawing holes and connections on that diagram, should the nominal diameter be indicated?
There’s nothing confusing; the calculations are correct, though they don’t always show up in the graph.
