The welder’s file should include: 1. Welder’s performance records: These records document the scope and quantity of welding work performed by the welder, as well as the periods during which welding work on supervised equipment (referring to boilers, pressure vessels, and pressure pipelines) was interrupted. Such records serve as a basis for determining whether the welder needs to take another exam before resuming welding work on supervised equipment after such interruptions. 2. Weld seam quality inspection results and welding quality incidents: These serve as the main basis for evaluating whether a welder is suitable to continue performing welding work on equipment under supervision, as well as for submitting applications for exemption from testing. 3. Welder training and assessment records: These are the records of the training provided to welders by the relevant organization, as well as the results of the assessments conducted. It also includes the following: 1. Welder’s resume 2. Copy of the welder’s ID card 3. Welder’s theoretical examination paper 4. Copy of the RT report for the welder’s test specimens 5. Copies of physical and chemical analysis reports 6. Copy of the welder’s certification 7. Record of the welder’s continuous operation 8. Welder’s work record sheet. Form for basic information on examinations for special equipment welding operators, Application form for examinations of special equipment welding operators, Record sheet for skill assessment examinations, High school diploma or above (including high school) (copy) or equivalent educational qualification proof ； A health certificate issued by a medical or healthcare institution, containing information on vision, color blindness, etc.
