The State Administration of Work Safety reported on the explosion incident at Xinjiang Yihua: its safety production license was revoked in accordance with the law. According to a report on CCTV.com, the State Administration of Work Safety announced recent information regarding production safety accidents that occurred at companies affiliated with Hubei Yihua (6.980, 0.00, 0.00%) Group. At around 18:06 on July 26, 2017, an explosion occurred in the gas gasification furnace of the ammonia synthesis unit at Xinjiang Yihua Chemical Co., Ltd., which is affiliated with Hubei Yihua Group Co., Ltd. (hereinafter referred to as Hubei Yihua Group). The company is located in the Zhundong Economic and Technological Development Zone in Changji Prefecture, Xinjiang Uygur Autonomous Region (hereinafter referred to as Xinjiang Yihua). The accident resulted in 2 deaths, 17 people suffering serious injuries (all due to burns), and 13 people with minor injuries. This was the fourth fatal accident to occur at Xinjiang Yihua this year. Currently, during the national safety inspection campaign, a series of accidents have occurred at Xinjiang Yihua, with significant impacts and a grave nature. On February 12 of this year, the company was ordered by the local authorities to suspend operations for rectification following an accident at its calcium carbide furnace that resulted in 2 deaths and 3 serious injuries ； On June 22, a contractor fell from a height during the shutdown for rectification, resulting in one death. According to preliminary statistics, the enterprises affiliated with Hubei Yihua Group have experienced a high frequency of production safety accidents in recent years. From 2011 to the end of July 2017, a total of 19 chemical accidents occurred, resulting in 23 deaths and 20 serious injuries ； A total of 3 coal mine accidents occurred (including one major incident that left 58 people trapped), resulting in 7 deaths. The successive accidents have revealed that Hubei Yihua Group and its affiliated enterprises have failed to adopt a safety-first approach to development; they prioritize growth over safety, exhibit a lack of awareness regarding safety, and practice rough and inadequate management. Systemic and structural flaws exist in all aspects, from engineering design and construction to daily safety management, and the primary responsibilities for ensuring safe production are not properly fulfilled. To thoroughly implement the important directives of the leaders of the State Council, and to urge Hubei Yihua Group and its affiliated enterprises to earnestly learn from the lessons of this accident, strictly fulfill their responsibilities regarding work safety, and effectively prevent and curb work-related accidents, the following requirements are put forward: 1. Severely investigate the responsible parties for the accident in accordance with the law. (1) Conduct a more in-depth investigation into the ’7·26’ fire and explosion accident at Xinjiang Yihua. In accordance with the relevant provisions of the Regulations on the Reporting, Investigation, and Handling of Production Safety Accidents (Decree No. 493 of the State Council) and the Notice of the General Office of the State Council on Strengthening Supervision and Law Enforcement in Production Safety (Guo Ban Fa [2015] No. 20), the Safety Supervision Bureau of the Xinjiang Uygur Autonomous Region has been instructed to request the local people’s government to establish an accident investigation team to conduct a more thorough investigation into the fire and explosion incident that occurred at Xinjiang Yihua on July 26. The accident investigation team must adhere to the principles of \"scientific rigor, seeking truth from facts, acting in accordance with laws and regulations, and focusing on practical results\", as well as the requirement of \"not letting go of any aspect related to the accident\". It is necessary to thoroughly investigate all the problems revealed by the accident; not only should the direct causes of the accident be analyzed, but also the underlying reasons for the successive occurrences of such accidents should be examined in depth. In particular, by taking into account the findings of the investigations into the first three accidents this year, it is essential to identify the systematic and structural issues existing within the companies involved in these accidents, and to deal with them strictly in accordance with laws and regulations. (II) Revoke the work safety license of Xinjiang Yihua in accordance with the law. Given the successive accidents that have occurred at Xinjiang Yihua this year, as well as the accident on July 26 that took place during the resumption of operations after the shutdown for rectification following the accident on February 12, it indicates that the company in question does not meet the basic requirements for safe production. The Safety Supervision Bureau of the Xinjiang Uygur Autonomous Region was instructed to revoke Xinjiang Yihua’s safety production license in accordance with the Regulations on Safety Production Licenses (Decree No. 397 of the State Council) and other relevant provisions, based on the findings of the investigation into the \"7·26\" accident. (III) Include Xinjiang Yihua in the “blacklist” for records of poor safety performance. In accordance with the provisions of the **Notice issued by the General Administration of Work Safety** (An Jian Zong Ban [2017] No. 49), and given that Xinjiang Yihua has experienced two production safety accidents this year that had a significant impact on society, once the investigation into the \"7·26\" fire and explosion incident at Xinjiang Yihua is completed, the company will be included in the \"blacklist\" of entities with poor safety records following the established procedures. The relevant information will then be transmitted to the Social Credit Information Sharing and Exchange Platform, enabling joint disciplinary actions by multiple departments. (IV) Conduct interviews with relevant units and personnel. The Hubei Provincial Work Safety Supervision Bureau has been instructed to, in conjunction with the People’s Government of Yichang City in Hubei Province, hold talks with the key officials of Hubei Yihua Group, urging them to firmly uphold the concepts of scientific and safe development, strengthen their awareness of critical limits, adhere to a bottom-line mindset, comprehensively enhance safety management, and effectively improve the level of work safety. The Hubei Provincial Safety Supervision Bureau should order the main responsible persons of Hubei Yihua Group to submit a sincere apology to the people’s government of Yichang City, Hubei Province. II. Further strengthen the safety supervision of Hubei Yihua Group and its affiliated enterprises (1) Strictly control the approval of construction projects carried out by Hubei Yihua Group and its affiliated enterprises. Given the frequent production safety accidents in recent years in various enterprises in the coal mining and chemical industries affiliated with Hubei Yihua Group, which reflect a serious deficiency in safety management capabilities on the part of this group and its subsidiaries, local safety supervision and inspection departments are instructed to request that the people’s governments at their respective levels suspend, in accordance with the law, the approval of new construction, expansion, restructuring, or mergers of high-risk projects, facilities, and enterprises by Hubei Yihua Group and its subsidiaries for a period of 3 years. (II) Immediately organize a comprehensive law enforcement inspection of the enterprises affiliated with Hubei Yihua Group. The safety supervision and inspection departments in the relevant areas must earnestly fulfill their responsibilities for local supervision, and further strengthen the supervision and inspection of Hubei Yihua Group and its affiliated enterprises. A review of the safety permits held by those enterprises that have obtained such permits should be carried out, as well as enhanced supervision and inspection both during and after operations ； Strict safety reviews must be conducted for the extension of safety production licenses for Hubei Yihua Group and its affiliated enterprises; such tasks must not be delegated to lower-level departments under any circumstances ； In conjunction with the ongoing national safety inspection campaign, an immediate comprehensive regulatory inspection should be carried out on the enterprises affiliated to Hubei Yihua Group within the jurisdiction, focusing on aspects such as automation upgrades and their implementation, management of major hazard sources, prevention of safety risks associated with special types of operations, development of safety management systems, training for key personnel and safety officers, and efforts to combat violations of safety regulations. At the same time, the enterprises affiliated with Hubei Yihua Group should be identified as key targets for comprehensive safety inspections conducted by the local people’s authorities and relevant departments; stricter daily supervision and oversight should be applied. In the event that serious issues are detected, penalties such as orders to suspend operations for rectification, suspension, or revocation of safety production licenses shall be imposed in accordance with the law. All regions should urge the relevant enterprises within their jurisdictions to earnestly learn from the series of production safety accidents that have occurred in recent years at enterprises affiliated with Hubei Yihua Group, firmly establish the concept of safe development, effectively assume primary responsibility for production safety, and comprehensively strengthen work related to production safety. At the same time, it is necessary to strictly follow the arrangements outlined in the national video conference on work safety held on July 21, 2017, and carry out thorough inspections of work safety conditions. Enforcement actions, penalties, and accountability measures should be strengthened to effectively prevent and curb serious work safety accidents, thereby creating a stable work safety environment for the successful holding of the Party’s * meeting. It has finally been found, so I’m sharing it with everyone: Approval regarding the Environmental Impact Assessment Report for Xinjiang Yihua Chemical Co., Ltd.’s project to produce 2×200,000 tons of ammonia and 600,000 tons of urea per year: Xinjiang Yihua Chemical Co., Ltd.: We have received your company’s request for approval of the “Environmental Impact Assessment Report for Xinjiang Yihua Chemical Co., Ltd.’s project to produce 2×200,000 tons of ammonia and 600,000 tons of urea per year”, along with the attached Environmental Impact Assessment Report (hereinafter referred to as the “Report”), the technical assessment report issued by the Autonomous Region Environmental Engineering Assessment Center (Xin Huan Ping Gu [2010] No. 231), and the comments provided by the Changji Prefecture Environmental Protection Bureau on the Report (Chang Zhou Huan Shen Han [2010] No. 41). After examination, the following approval is given: First, the project of Xinjiang Yihua Chemical Co., Ltd., which aims to produce 2×200,000 tons of synthetic ammonia and 600,000 tons of urea per year, is located in the Wucaiwan Industrial Zone, within the western part of the coal, electricity, and coal chemical industry belt in the Zhundong region of Xinjiang. The proposed site for this project is approximately 97 kilometers north of the county seat of Jimsar County, and about 7 kilometers southeast of the industrial complex of Yihua Wucaiwan open-pit coal mine. This is a new project that uses coal as raw material; syngas is produced through coal gasification technology. Hydrogen is obtained via processes such as reforming, low-temperature methanol washing, and methanation. Liquid ammonia is then produced through the ammonia synthesis process, and finally urea is manufactured using liquid ammonia as the raw material. The scale of this project is an annual production of 400,000 tons of synthetic ammonia (with two plants, each with a capacity of 200,000 tons per year), 600,000 tons of urea, and 2,112 tons of sulfur as a by-product per year. II. The main construction components of the project include: a coal preparation system, a coal gasification system (using BGL coal crushing and slag gasification technology as well as oxygen-enriched continuous gasification technology for lump coal), a coal gas purification system (employing sulfur-resistant shift reaction, low-temperature methanol washing processes, and methanation technology), an ammonia synthesis system (using a 31.4 MPa synthesis process), and a urea production unit; a power station (2×220 t/h circulating fluidized bed boilers, 2×25 MW back-pressure steam turbine generators); utility and auxiliary facilities (air separation units, flares, refrigeration plants, ash storage areas, etc.); and environmental protection systems (treatment of coal gasification wastewater, sulfur recovery, sewage treatment systems, etc., as well as electrostatic dust removal for the boilers in the power station and limestone desulfurization inside the boilers). III. Based on the evaluation conclusions of the Report, the technical assessment opinions contained in the Report, and the review opinions of the Changji Prefecture Environmental Protection Bureau, it is principally agreed, from the perspective of environmental protection, that the project shall be constructed in accordance with the nature, scale, location, production processes to be employed, and environmental protection measures specified in the Report.
According to the approval documents, the gasification technology makes use of BGL and atmospheric-pressure continuous fixed-bed reactors; it is not clear which manufacturer’s reactor was used in the gasifier that experienced this explosion.

Two major accidents have already occurred successively at Yihua in Xinjiang and Hubei; it seems that the only option is to wait for other companies to acquire it. There are serious problems with management; all the former managers should be added to the blacklist of the chemical industry.

Major changes in the leadership team are a way of deceiving the people; those from Yihua can’t take on roles in Xinhua, and those from Xinhua can’t take on roles in Nenghua. It’s the frontline employees who are truly pitiful: staying means work is unlikely for a long time, while leaving means they lack any special skills.
Yes, I can fully understand this situation; it’s a difficult choice. For a company, one poor soldier is just one poor soldier, but bad leaders can lead to an entire group of poor performers. Without a strong leadership team, there is no future or hope.
