As the title suggests, does anyone have any accident cases that occurred during pressure testing? A compiled version would be better. Since a pressure test is upcoming, I want to provide some training for certain staff before the task begins. I searched many websites but couldn’t find any systematic information on this topic, so I had to turn to everyone for help. Thank you! ! ! ! ~~~~
There are no cases, but I have seen one pressure testing case before. Perform a pressure test on a gas cylinder. Before conducting the pressure test, the maintenance workers said that a pressure of 0.4 MPa should be used, but the supervisor insisted on 4 MPa. In reality, what is meant by \"pressure\" in this context is usually 4 kilograms of force. Problems arose during the communication process; if we had followed the maintenance workers’ instructions, no issues would have occurred. But by following the supervisor’s orders and increasing the pressure from 4 kilograms to 40 kilograms, the container ended up exploding. The consequences are obvious: one death and several injuries. . . There’s no need to elaborate; a pressure testing plan must be prepared before conducting the pressure test. . .
