Cause analysis of steam heater leakage

Our factory has two regenerative steam heaters, one in use and one as a backup. Today, I found that the backup heater is leaking. The dew point was within acceptable limits before being used as a backup. Upon disassembly, it was found that there was a crack in the inlet manifold of the tube bundle for condensate flow. I would like experts to analyze it for me. Thank you.
The reasons are: 1. The design structure is unreasonable； 2、Poor production quality ； 3. Operational issues. The heating rate was too fast, causing uneven heating of the pipeline and resulting in its expansion and rupture.
Is there no room for expansion and contraction when it gets hot or cold?
