Purpose: To establish a team whose members possess knowledge of processes/products, have the necessary time available, and have been granted the appropriate authority; they should also have the technical skills required to solve problems and implement corrective actions. The group must have a supervisor and a team leader. Key points: Membership, knowledge of processes and products ; Goal ; Division of labor ; Program ; Team Building D2: Problem Description Purpose: To detail the internal/external customer complaints related to this issue using quantitative terms, such as what happened, where, when, to what extent, and how frequently. ““What went wrong?” Method: Quality risk assessment, FMEA analysis. Key points: 1. Collect and organize all relevant data to explain the problem. 2. A problem description is a summary of data that is particularly useful for describing the issue at hand. 3. Review existing data to identify the problem and determine its scope. 4. Break down the problem by dividing complex issues into individual ones. 5. Define the problem by finding a description that matches what the customer has identified as the problem; in other words, “What went wrong?”, while the cause remains unknown, along with the risk level. D3: Implement and verify temporary measures. Purpose: To isolate the issue from internal and external customers until permanent corrective actions are taken. (Originally the only optional step, but it is now required in all cases.) Methods: FMEA, DOE, PPM. Key points: 1. Evaluate emergency response measures. 2. Develop a control plan. 8D problem-solving approach: D5 – Select and verify permanent corrective actions. Purpose: Test the solution before production and review it to ensure that the chosen corrective actions can resolve the customer’s issue without having adverse effects on other processes. Methods: Key points of FMEA: 1. Re-examine the qualifications of team members. 2. Make a decision to select the best corrective action. 3.
