Leakage at the flange of the internal heat extraction outlet valve, as well as scaling issues

Steam is leaking from the lower flange of the internal heat extraction outlet valve, and a thick layer of scale has formed around the flange; it’s white in color, like lime. I’m not sure what causes this! (The leader suspects that it was caused by too little trisodium phosphate added); I hope fellow sailors can offer some advice.

Has there been any analysis of the silica content in the steam? This is likely due to high water hardness, which in turn results in high steam hardness; it’s probably related to trisodium phosphate, but not to a great extent
