The direct cause was that the workers did not use any protective measures during their work, which led to a wrench being thrown and injuring someone; the indirect cause was that Hu and the other workers failed to adhere to the principle of avoiding harm to oneself, others, and the environment. It is necessary to strengthen safety education and awareness among employees, and at the same time, better protective measures as well as enhanced supervision should be implemented during construction activities.
A supervisor should be assigned for work at heights; no one should be below the working area. Tools should be kept in tool bags, and wrenches used for striking should be tied with a rope to prevent them from falling and causing injury. The implementation of safety measures in these stages should be supervised and checked by the team leader, supervisor, and safety officer.
