Looking at the image, it appears that there is a crack in the hard PTFE layer; the corrosive liquid penetrated into the separator layer, causing corrosion. This is likely due to a quality issue with the lining itself, or irregularities in the flange sealing surfaces, as well as improper selection of gaskets that led to compression and deformation of the lining, resulting in cracking.
There are problems with the flange flanging; even the use of gaskets cannot prevent liquid from entering the lining and the gap between the carbon steel pipes. Especially with such a high temperature, any difference in temperature can have an impact. Try using a gasket with a smaller inner diameter and wrapping more tape around it!
Based on an analysis of the image: there is a problem with the lining material; it’s possible that the wrong material was chosen. Bubbling occurring at a temperature of 90° indicates that the material is likely not F4. If it were F4, then there would be serious issues with the quality of that material, as its tensile strength and ductility would be significantly insufficient; The cracking of the sealing surface at port R due to the butting of the two pipes indicates that the manufacturer failed to take stress concentration into proper consideration.
