This is likely due to stress causing it to be pulled and deformed, resulting in damage. I suggest you check again the direction of the forces acting on this tee, and see if there are issues such as pipe supports being improperly installed or the tee being used as a support point. Let’s take a look from a mechanical analysis perspective. When machining tees, cracks should appear in the area with the greatest deformation; they should be transverse cracks. As a supplementary point, the vibrations generated by the pipeline during normal operation can also cause this area to tear if additional components are installed in an inappropriate manner. (Personal opinion, for reference only)

The cracks are very regular; is it related to manufacturing defects? Do you think this cracking is related to the manufacturing process? The red line indicates the direction of the tee.
