Why is there frequent internal leakage in the tube bundle of the vaporizer heat exchanger for hydrocracking tail oil?

In our facility’s hydrocracking unit, the tail oil heat exchangers have experienced frequent tube bundle perforations over the past two years; we have no choice but to plug the leaks as they occur. Have other colleagues encountered such a problem? Tube side: Medium – hydrocracking tail oil, temperature ranging from 220 degrees to 150 degrees. Shell side: Medium – deoxygenated water; produces steam at 0.45 MPA. Could any colleagues help with an analysis? If more data is needed, it can be provided further!
It was damaged either by corrosion or erosion; it is recommended to use a better material.
What type of heat exchanger is it? Are the main perforations concentrated?
