The heat exchanger on the platform had been in use for less than 10 years when its heat exchange tubes suddenly broke and started leaking water, causing heavy flooding throughout the platform and creating a complete mess. Several days later, when the core was removed, it was found that the heat exchange tubes had broken in multiple places, and this situation did not correspond to what was described in various documents regarding the type of failure that would occur.
