Leakage occurred in the organic base solution reboiler after about 2 years

The organic base solution reboiler has been in use for two years; the operating temperature is 140°C, which is above the boiling point of the solvent. Leaks occurred after just 2 years of use – could someone please explain the reason?
Where is the leak? Welded joint or tube? Welding or material issues
The solution inside the reboiler itself will boil within the pipes; one reason is the material； The second is the manufacturing process
