Before leaving, the person handing over duty must, in addition to carrying out routine maintenance tasks, carefully record in the \"Shift Handover Log\" details such as the operation status of the equipment during their shift, any problems encountered during operation, repairs carried out for faults, and any matters that require attention by the person taking over. They should also provide a thorough briefing to the incoming staff, conduct a joint inspection, and sign the log. If an in-person handover is not possible, the person responsible for the handover can carry out routine maintenance to keep the equipment in a safe condition, fill out the handover report, and submit it to the relevant supervisor for signature. If the successor detects any abnormal conditions in the equipment, finds that the records are incomplete, or observes that the equipment has not been maintained as required, they may refuse to take over the shift. If problems occur with the equipment after it is taken over as part of the shift handover, the person who takes over is responsible. Equipment management and maintenance personnel should regularly check the implementation of the shift handover system, review the shift handover records at any time, analyze the technical condition of the equipment from them, and provide information for condition management and maintenance.
Shift handover system I. Handover 1. The shift supervisor handing over the shift should conduct a thorough inspection of all equipment, pipeline valves, instruments, etc., one hour before ending his/her shift; any issues found must be resolved promptly, without delaying the next shift. 2. 30 minutes before shift handover, the operator on duty should wipe down the machines, electrical equipment, and instruments in their assigned area. Organize the records, clean the work area, check tools, instruments, protective equipment, fire-fighting gear, etc., and complete all preparations before handing over the shift. 3. Before handing over the shift, the current crew member should provide the incoming crew member with a detailed account of the situation during that shift, ensuring ten aspects are covered, namely: ①. The production status during the shift ; ②. Implementation status of process parameters and existing issues ; ③. Causes of the accident, handling procedures, and outcomes ; ④. Equipment operation and maintenance status ; ⑤. The storage and use of instruments, meters, and tools ; ⑥. Status of protection for form filling ; ⑦. Indoor, outdoor, and equipment hygiene ; ⑧. Leakage, spills, and the usage of machine oils ; ⑨. Safety production status ; ⑩. Leadership instructions ; Six no-deliveries, namely: ① Delivery is not allowed if the process parameters do not meet the requirements ; ② If the equipment is not properly maintained, no payment will be made ; ③ No release until the cause of the accident is determined ; ④ No delivery if tools are incomplete ; ⑤ If the record sheets are incomplete, untidy, or not submitted ; ⑥ Equipment and environmental hygiene that are not clean will not be accepted ; II. Shift Handover 1. The person taking over the shift must arrive at the post 10 minutes in advance, first conducting a routine inspection of the area by checking the equipment, electrical instruments, and the status of production operations. 2. Key inspection points: Verify whether the ten requirements and six prohibitions are met; check the operating status of valves on the equipment, the proper use of instruments, as well as the normal functioning of oil, water, and electricity systems; ensure that temperatures, pressures, liquid levels, flow rates, etc., at all control points meet the process requirements. 3. Adhere to the five don’ts: ① Do not take over if the items to be handed over are not clearly explained ; ② Do not connect if there are safety hazards ; ③ The cause of the accident is unclear; no response will be given until it is resolved ; ④ The device does not connect when it is operating abnormally ; ⑤ Incomplete tools, equipment, and a messy/unorganized work site ; III. Formation: The shift-handover and incoming shift teams should form up separately, ensuring they are properly dressed and in uniform. They must listen carefully to the briefing on production conditions given by the person in charge of the previous shift, as well as to the instructions regarding production tasks given by their own shift leader. IV. Handover: During the shift handover, it is necessary to explain in detail to the person taking over the operation and maintenance status of the equipment during the current shift, and to fill out relevant records. If a dispute arises between the two parties during the handover, it should be reported to the on-duty staff promptly for resolution through consultation. During this period, the person handing over the shift shall operate normally until the problem is resolved ; Accidents that occur during the handover period are the responsibility of the person handing over, while accidents that occur after the handover are the responsibility of the person taking over.
