Accident caused by improper operation: the collapse of the condensate tank. Incident details: On October 13, 2000, the water seal tank for one of the condensate tanks was purged with steam for over 24 hours. Around 10:00, the steam supply was stopped and the drain at the bottom of the tank was slowly opened. To prevent air from entering the condensate tank, water was poured into its water seal tank; as a result, the liquid level in the tank began to drop, creating negative pressure that caused the condensate tank to collapse. Cause of the accident: a. After scanning, fresh water was injected immediately when the temperature inside the tank was high, resulting in a drop in tank pressure; a negative pressure formed inside the tank, which caused it to collapse. b. The organizational coordination for workshop process management is inadequate, and the purging plan is not detailed or specific enough. c. The quality of operators is low, and management provides inadequate supervision. Lessons learned and preventive measures: a. Conduct comprehensive accident anticipation in advance during maintenance work ; b. Carefully examine each operation during maintenance to ensure everything is error-free ; c. Strengthen education on the technical skills and sense of responsibility among staff, so as to improve their operational proficiency and enhance their sense of accountability.

We suddenly encountered a downpour; when the steam was stopped, the tanks immediately deflated, and we tried that approach. Therefore, nitrogen injection can be considered before stopping the steam.
There are no corresponding operating procedures; people just act recklessly
