Dear seafarers, what information is included in your company’s shift handover logs? What needs to be handed over when submitting and receiving something? Welcome to any suggestions and discussions; thank you~~~
Understanding the principles of ’five deliveries and five non-deliveries’ in chemical production
5.1 Shift handover principles 5.1.1 Shift handovers must be established for positions that operate continuously, and a shift handover log must be kept. 5.1.2 Both parties involved in the shift handover must adhere to the principles of “ten handovers” and “five non-acceptances”. 5.1.2.1 “Ten Deliverables”: delivering tasks and instructions, delivering operations, delivering targets, delivering quality, delivering equipment, delivering safety, delivering environmental and hygiene standards, delivering problems and experiences, delivering tools, and delivering records. 5.1.2.2 “Five No’s for Acceptance”: Do not accept if the equipment is not properly lubricated; do not accept if the tools are incomplete; do not accept if the operating procedures are not clearly explained; do not accept if the records are incomplete; do not accept if the hygiene standards are not met. 5.1.3 Pre-shift inspection. The successor must arrive at the factory 15 minutes in advance to conduct a pre-shift inspection of the systems on site, and attend the handover meeting on time. 5.2 Holding the shift handover meeting 5.2.1 All personnel taking over the shift and the shift supervisor must attend the shift handover meeting ; The directors and deputy directors of various grassroots departments, as well as process and equipment technicians, must attend the shift handover meeting. 5.2.2 The shift handover meeting begins with the outgoing shift leader outlining the production status for the current shift, the completion of work tasks, as well as any issues that remain unresolved or need to be addressed. The incoming shift leader and team members raise relevant issues based on the preliminary inspection results and discuss them with the outgoing shift leader to find a solution. 5.2.3 The department head and other management personnel, or the personnel on duty on that day, shall put forward specific requirements based on the handover situation, the actual production conditions of the equipment, and the tasks at hand, such as precautions for production and requirements for handling any outstanding issues. And instructions are given regarding safety matters in the operation process. 5.2.4 The shift leader assigns the tasks for the shift and relevant precautions. 5.2.5 For special job positions, comprehensive handover by the team leader as well as targeted handover between different positions can be implemented. 5.3 Shift handover requirements 5.3.1 The parties involved in the handover must conduct it in a serious and thorough manner, based on facts, and ensure clear communication face to face. 5.3.2 After the handover is completed, both parties involved sign the “Shift Handover Log”. Any problems that occur during production after signing shall be the responsibility of the person taking over. 5.3.3 The person taking over must not leave their post until the person handing over has signed in. 5.4 Requirements for filling out the shift handover log 5.4.1 Main tasks of the shift handover: Restate the key process operation parameters and any issues left unresolved from the previous shift. 5.4.2 The main tasks of this shift include: the production load of the equipment, any changes in the production plan (including the timing of such changes and alterations to the key process parameter values), as well as any corresponding adjustments to the flow direction of fluids; adjustments to important valves; the operating status of the equipment (including actions taken regarding equipment that needs maintenance); the causes and resolutions of fluctuations, abnormalities, and failures that occur during operation; problems identified during inspections and the actions taken to address them; the status of interlock systems in use; and compliance with the production instructions and requirements set by the company’s production control department and other management levels. 5.4.3 Tasks to be handed over to the next shift: including current production instructions, production plans, and key operating parameters ; Device status (there are unresolved issues that need to be addressed) ; Fire safety, security, hygiene, and the condition of tools, etc. 5.4.4 The entries in the shift handover log must be written in neat and clear handwriting, with a tidy layout; efforts should be made to use the Fangsong script style, and such logs shall be managed in accordance with quality record standards. Timely, detailed, and accurate records must be kept of the execution and completion of various production tasks during the shift, as well as any adjustments made to operations; fraud is not permitted. Detailed records should be kept of the handling processes, countermeasures, and cause analyses for various abnormalities that occur during production, such as parameter exceedances, production fluctuations, alarms, activation of interlocks, and other such issues. 5.5 Other requirements 5.5.1 In principle, no major changes or adjustments should be made to the production plan and process conditions one hour before shift handover. 5.5.2 In the event of production issues during the handover period, the person handing over the shift shall take primary responsibility for handling them, with the person taking over providing assistance until they deem it possible to assume control.
I. Tasks to be carried out during shift handover 1. Arrive at the control room on time to participate in the shift handover, listen to the production status reported by the previous shift supervisor as well as the production instructions from the warehouse management, and then take over the post on time. 2. Change into protective clothing after taking over the post. 3. Carefully examine the information in the daily shift handover logs and production records regarding the operation of environmental protection equipment (oil and gas recovery, wastewater treatment), the operation of liquid nitrogen facilities, the inflow and outflow of oil in storage tanks, any changes in processes, expansion of pipelines and storage tanks, water removal from storage tanks, the operation of pumps and machinery, activities at the dock, as well as any instructions from management. 4. Together with the person handing over the shift, inspect the site along the scheduled inspection route to verify that it matches the situation as described in the handover report, and carry out a proper handover at the production site, strictly adhering to the \"Ten Handovers\" and \"Five Non-Acceptances\" rules. 5. After the handover is complete, the incoming shift worker must sign the handover log before the departing shift worker can leave their post. The successor records the handover details in the \"Handover Log,\" while the progress and data related to the handover are recorded in the \"Production Record Book.\" II. Tasks to be carried out in the class: 1. Inspect the environmental protection devices as required. Monitor the operation of the liquid nitrogen facility and record relevant process parameters on time. 2. Record the dynamics of the oil tank, process changes, start and stop times of its status, as well as the oil tank level (data from instruments and manual measurements), in the order of production changes. 3. Continuously monitor changes in the liquid level of the oil tanks; record the levels at specified intervals, and verify the data for both moving and stationary tanks. If any abnormalities are detected, promptly investigate the causes and make corresponding reports and records. 4. Conduct gauge readings on the relevant oil tanks in accordance with regulations. 5. Perform process conversion and review as required. 6. Carry out terminal operations such as cable handling, mooring, unmooring, operating the oil transfer arm, and lifting hoses in accordance with regulations. 7. Request the shipowner for the ‘Declaration Form for Ship Carrying Dangerous Goods’ issued by the maritime authorities. The ship’s crew and terminal operators jointly inspect the ship’s and shore’s safety facilities, and fill out the \"Ship/Shore Safety Inspection Form\" based on the findings. 8. Conduct inspections as required. 9. Check the condition of the fire protection and environmental protection facilities in this position. 10. Maintain the cleanliness of the operation room and changing room for this position. III. Tasks to be carried out during shift handover 1. Record the production changes of the current shift in the \"Shift Handover Log\" 30 minutes before the handover. (1) Startup and shutdown of environmental protection equipment. (2) Operation status of liquid nitrogen facilities. (4) Water separation from oil tanks. (5) Issues identified during inspections. (6) Ball passing treatment status of pipelines. (7) Abnormal situations that occur during oil receipt and delivery. (9) Instrument failures and repair status. (10) Uncompleted tasks. (11) Inspection fault records. 2. Before handing over the shift, the production status should be accurately recorded in the Production Record Book. 3. Cooperate with the incoming staff to carry out a proper handover at the production site; once the handover is complete, the incoming staff shall sign the Handover Log before the outgoing staff can leave their post.
Based on the specific circumstances of your position, you can refer to the shift-handover management system and make further improvements as needed.
