This is rare; firstly, the boiler operators are highly skilled, and secondly, the coal used is of better quality than what was specified in the design.

I have encountered this situation as well, but it was in a coal powder furnace; the coal quality was good, the fly ash was very white, and the efficiency obtained from reverse balance calculations was higher than that designed
