What happened: Around 13:20 on July 19, after the methanol circulation pump was started up for testing, the temperatures of sensors TIA2220 and 2221 in the gearbox rose rapidly to 150°C. The plant’s representatives believed that these were false signals and that there was no need to stop the machine; however, the process engineers failed to shut it down in time, which resulted in damage to the bearings. Reasons: 1. Lack of awareness regarding the protection of critical pumps, insufficient operational experience, and indecision in critical situations. 2. Failure to report abnormalities promptly, which delayed timely handling
Sometimes, there are indeed more issues with the instruments. If it’s a problem with the instrument panel and you fail to detect it, stopping the vehicle will have negative consequences.
