What are the VCM near-miss accidents?

Are they attempted incidents related to the VCM monomer, or those that may involve various raw materials in this facility? Or is it a hardware issue? ?

During the cleaning of a furnace, the worker responsible for cleaning was locked inside it; he got so scared that he urinated himself, and then he started hitting the door with a hammer desperately until he was finally found and rescued； 2. During the early shift, the shift leader asked an employee to open the manhole of reactor B. That guy was probably still asleep; he went to use the tools to open reactor D, which was in the process of reacting. Fortunately, a sober employee on site stopped him in time, and everyone was terrified afterwards ； 3. A fire broke out in the calcium carbide storage area, and a guy rushed in there carrying a big bucket of water. Fortunately, he wasn’t strong enough; he was stopped before even reaching the entrance to the storage area. Everyone laughed uncontrollably afterwards: lol:lol:lol

I have experienced several of the things mentioned upstairs; I’ll add a few more. 1. The vinyl chloride converter was taken out of service, and the hot water valve of the converter that was operating nearby was closed. 2. During the maintenance of the acetylene generator, the instrument air supply for the operating generator was mistakenly turned off, causing the valve in the generator’s storage tank to open on its own.
