It’s probably due to insufficient wall thickness at the joint. If reinforcement for openings is calculated using the equal area method, the calculated thickness of the pipe joints must first be determined based solely on internal pressure ； If it fails here, further calculations won’t be possible.
Perhaps you didn’t meet the requirements in the first place; for example, you should have used a welded branch fitting platform, but you used a socket-welded one instead. An eccentric tee should be used; you used welded branch fittings and so on...
