Questions regarding reverse osmosis systems

What are the causes of the following problems in reverse osmosis? Process: Raw water → Raw water pump → Sand filtration → Carbon filtration → Safety filter → High-pressure pump → Reverse osmosis system. Problem: 1. The outlet pressure of the safety filter is higher than the inlet pressure. 2. The pressure between the reverse osmosis membranes is higher than the pressure of the water output from behind the membranes. (The reverse osmosis system has three pressures in total: before the membrane, between the membranes, and after the membrane.)
Our plant’s process is: raw water → raw water pump → multi-media filter → disc filter → ultrafiltration → ultrafiltration tank → pressure pump → safety filter → high-pressure pump → reverse osmosis system
Are all changes in pressure caused by blockages? Is the filter element or membrane clogged?
