Because construction companies always build according to the drawings; they won’t add anything that isn’t specified in the drawings, as they would charge extra for any subsequent changes. Secondly, the proper approach for this design is to create an opening at the top of the straight ladder, with protective railings surrounding that opening; at the points where people enter and exit, soft chains should be in place to prevent them from falling accidentally. If the operator wants to go down, they need to remove the chain in order to do so. To meet the deadline, construction was completed before the equipment had been finalized; really, it can’t be blamed on the design institute.
