Systematic analysis of accident causes

My humble opinion: \"When an accident occurs in one factory, all factories learn from it; when an accident happens in one region, the whole country takes warning.\" This can’t be considered experience at all. On the contrary, the approach of having everyone take medication just because one person is sick is unsuitable, and it only serves to increase people’s indifference towards safety and the identification of potential hazards. Because often, the accident units themselves do not yet understand the causes of their accidents – they are unclear about what to investigate, how to conduct the investigations, and how to prevent such accidents; let alone other companies. So this approach, which appears to trigger an alarm across the entire industry, is merely a show of force that actually yields poor results; it is even, to a large extent, a mere formality resulting from unclear objectives, thereby exacerbating complacency in safety management!
