How are holes made in the pressure vessel head?

What processing technique is used for making holes in the pressure vessel head? Drill press? Lathes? Milling machine? Or some other process?
Plasma cutting; small end caps can be turned on a lathe
It depends on the size of the hole, I guess? If a flame cutter can be used, go with that; for small holes, drilling is the best option.
It is usually done by gas cutting or plasma cutting
I’m also aware of those all-purpose grinders, but the problem is that the holes are too small, making it difficult for the grinder to work properly!
What about the small ones? Won’t gas cutting affect the strength?
