(3) Documents (copies) establishing a work safety management organization and assigning work safety management personnel ； (4) Qualification certificates and operation permits for key personnel, those in charge of safety, and safety management staff (copies) ； (5) Documents (copies) regarding the regulations on the allocation and use of safety production expenses ； (6) As proof of payment of work-related injury insurance premiums for employees, in principle, there should be proof of payment of work safety liability insurance (a copy) ； (7) Certificate of filing for the emergency rescue plan for hazardous chemical accidents (copy) ； (8) Hazardous chemicals registration certificate or relevant registration documentation (copy) ； (9) Copy of the business license or official approval document from the industrial and commercial authorities (copy) ； (10) Letter of approval for the completion acceptance of safety facilities in the construction project and permission for commencing production (use) (copy) ； (11) List of emergency rescue organizations or personnel, as well as emergency rescue equipment and facilities. (12) Enterprises with major hazard sources shall also provide proof materials (copies) for the registration of such hazard sources.
