A 2-cubic-meter sulfuric acid fuming tank had its lid explode during the pumping of acid; it was flung 10 meters away and the acid sprayed onto the operator. May I ask what caused the explosion of this tank? Thank you.

The liquid is pumped from the large tank into the metering tank using a pump, and then from there into the reaction vessel. During the pumping process into the metering tank, the tank lid ruptured and flew off, causing acid to spill and injure people. What causes the metering tank to explode during pump delivery?
Is the breathing valve clogged or rusted?
There is only a drain pipe, no breather valve.
