The shell-side pressure surged to 1.1 times the design value, posing a risk of pipe rupture due to overpressure; it must be addressed within 10 minutes, with the key actions being pressure relief and identifying the cause. Potential hazards: ① Overpressure deformation of the shell in the shell side (ellipticity exceeds 5% of the design value, making repair impossible) ; ②The bolts connecting the tube sheet to the shell broke, resulting in a massive leakage of the medium ; ③Overpressure injection of high-temperature media can easily cause fires and burns. Emergency measures: ① Immediately open the bypass valve adjacent to the shell-side safety valve (or emergency pressure relief valve) to reduce the pressure below the design value ; ②Cut off the heat source on the shell side (such as closing the steam inlet valve and the hot oil inlet valve) ; ③Identify the cause (if it is clogging due to scaling, stop the machine for cleaning; if it is a valve failure, replace the valve). Example: In a certain project, the pressure on the shell side of the ammonia cooler increased suddenly. After taking corrective action, the pressure dropped to a safe level within 15 minutes, and no damage to the equipment occurred. Subsequent investigations revealed that internal leakage in the outlet valve had caused the blockage.
