The circulating water in the heat exchanger flows through the shell side, and due to the poor water quality, scaling occurs leading to blockages. Using chemical cleaning does not yield very good results. The photos before and after cleaning are shown below

For such operating conditions, a floating-head or U-tube heat exchanger should be used, with the heat exchange tubes arranged in a square pattern. Replace the heat exchanger, and use chemical and mechanical cleaning for the tube bundle.
What is your hot material? With such poor quality of the circulating water, it might be advisable to use the circulating water in the tube side.

First, carry out chemical soaking; find a way to extract the core. After extraction, apply chemical soaking again, along with high-pressure flushing. Repeat this process several times – there is no better solution. Be sure to pay close attention to the ratio of the solutions used, and conduct tests on the samples during the soaking process; the pipes must not become permeable before the scale has been removed!
