Which of the measures specified in Document No. 116 issued by the State Administration of Work Safety has your organization implemented?
Isn’t a 116 file just a file that, if not executed, will result in the closure? How can it not be executed?

HAZOP is a Hazard and Operability Analysis that identifies potential operational abnormalities that may occur during operation, as well as the risks they might pose. It is necessary to analyze the existing protection/reduction barriers, and determine whether the risk is acceptable based on the acceptable level of risk, as well as whether design changes are required to eliminate the risk. It specifies that the SIL levels range from 1 to 4. 61511 is applicable to the process industry and is specifically designed for the management of safety instrumented systems, specifying the specific requirements for IPF.
