Methyl diphenyl ether was previously produced in the production facility; the packing in the crude product distillation column needed to be replaced, and the crude product distillation was carried out using vacuum distillation. Once a batch of material has been processed in the crude distillation kettle, the residues are removed; after filling the kettle with water, the tower is immediately disassembled, without being rinsed with water or steam. The lowered tower was placed horizontally on the second floor; since it was already after working hours, everyone hurriedly packed up their tools and left work without checking the condition of the tower. On the second day of work, I prepared to remove the original filler and install a new one. But when I got to the scene, it was terrifying – the inside of the tower was burning brightly red, and even the filler had turned red. Extinguish the fire immediately and use steam for flushing. Upon analysis of the reasons, it was found that the tower body had not been flushed, leaving high temperatures inside it; the liquid-holding capacity of the packing, combined with an adequate supply of oxygen, led to a fire breaking out. At that time, everyone thought that since the filler was going to be replaced anyway, there was no need to rinse it, but cutting corners once almost led to a major accident.
Details in production cannot be ignored! Details during maintenance cannot be ignored, and the same care must be taken with the details involved in repairs and modifications!
