Problems with duct tunnel ladders

There is a work platform on our plant’s utility tunnel, 9 meters high. It needs to be used multiple times a day, going up and down. It uses a straight-up-and-down \"eye\"-shaped ladder with guardrails; I’m really worried that one day I might lose grip or the rebar could come loose and cause a fall. I think for a commonly used climbing ladder of this height, it should have a step structure similar to that of stairs. Please share your opinions; it would be great if you could provide the sources for any relevant standards!
An inclined ladder is often used for operations, instead of a regular ladder.
For areas that are used frequently, an inclined ladder is advisable.

If it needs to be moved up and down multiple times a day, why not design it in a zigzag shape?
The standard requires a platform when the height exceeds six meters; 9 meters is not acceptable either.
