Have the relevant regulatory agencies truly taken action after each accident to prevent similar incidents from occurring again? Furthermore, how to integrate environment and development should be a matter for consideration by the local leaders of each chemical industrial park; it cannot be simply ignored.
The 13 corrective actions required by Tianjiayi are as follows: 1. The key personnel have not passed the assessments on safety knowledge and management skills; 2. Only one person among those engaged in special instrument-related tasks holds the necessary certification, which is insufficient to meet the actual needs of safe production; 3. The operating procedures for the production facilities are incomplete – there are no procedures specific to the benzene storage area nor any process technical specifications, and there is no system for routine inspections with clear requirements for such inspections; 4. After interlocks were installed in the nitration facility, the operating procedures were not updated or revised in a timely manner; 5. Some of the DCS and SIS pressure transmitters used in the dinitration reactors share the same pressure sensing point; 6. Emergency shut-off valves at the base of the tanks in the benzene and methanol storage areas, which constitute secondary major hazard sources, are not installed; 7. The shut-off valves for the hydrogen supply lines in some dinitration reactors are connected to bypass lines, and the interlocks are not functional; 8. The cabinet rooms and monitoring stations are located inside the nitration plant, which violates regulations; 9. The safety responsibility systems for certain positions do not match the actual production conditions of the company – for example, the procurement department does not set any requirements regarding the safety and quality of purchased products; 10. On-site management is poor, with frequent leaks; there are insufficient safety warning signs, some of which are unclear, and there are no wind direction indicators on site; 11. The management of hot work operations is not proper – for instance, some safety measures lack designated persons responsible for verification, and the results of flammable gas tests are recorded as “none” or “no flammable gases”; 12. There are no emergency measures in place to deal with leaks at the sites where benzene and methanol are loaded and unloaded. The filling points are located too close to the pump areas, the eye wash stations on site are damaged and lack water; 13. The operators interviewed on site were unaware of the settings for flammable gas alarms in the facility, nor of the emergency procedures to be followed in case of an alarm. The flammable gas alarms in the nitration workshop do not have visual alarm functions.
After it is shut down, how to deal with the soil and whether the factory needs to be relocated are decisions that are like cutting off one’s own arm in a act of determination. There are still many more who need treatment.
Shutting down chemical industrial parks merely means losing one source of income and a few migrant workers; it does not affect performance metrics.
