How can we prevent the following inconsistencies from occurring? 1. Inconsistencies within the completion documents themselves: There is a pipeline labeled A in the process diagrams and PID charts, but it is not present on site; upon checking the piping diagrams, it is also not there. In other words, the PID charts do not match the PDMS diagrams nor the actual construction on site. 2. Inconsistencies between the interlock logic diagrams, configuration data tables, and process parameter values: In the interlock logic diagrams and configuration data tables, the high-high, low-low, high-alarm, and low-alarm values for the same tag number do not match each other. Moreover, these values also do not correspond to the alarm and interlock values within the SIS system. Additionally, they do not match the process parameters (such as the catalyst design temperature or process design temperature). For example, in the configuration data table, the interlock value for reactor temperature high-high, TSHH1234, is 330°C, while it is 355°C in the logic diagrams; after completion, the interlock value within the SIS system is 377°C, and the process parameter (catalyst final temperature) is 410°C. It’s all too confusing – it’s impossible to make sense of it. 3. The completion drawings are signed by relevant personnel, and they go through three levels of approval: design, verification, and review. Then why weren’t these issues detected? 4. How can we prevent the above problems from occurring to the greatest extent possible? 5. Management of design changes: How to ensure consistency across various specialties and prevent constant changes that end up with everyone on different pages?
Is this chaos due to subcontracting or something? Basically, the design standards specified in the technical agreement should be followed; otherwise, one will have to bear the consequences.
