As is well known, a lack of lubricant in moving equipment can lead to serious consequences; the most immediate issue is bearing wear. What are the worst accidents that have occurred due to inadequate lubrication that you have seen or experienced?

I’ve seen the gauge shaft; the false liquid level is indicated by the lubricating oil window.
In the mixing reducer, a lack of lubricating oil caused the bearings to burn out, leading to shutdown.
Sudden power loss; no shaft-mounted pump, shaft seized.
Due to a fault in the external power supply, all auxiliary equipment lost power (lubrication oil pumps, cooling fans, cooling water systems). The control for the high-voltage power supply is operated by low-voltage power; in other words, without low-voltage power, the shutdown button for high-voltage power cannot be used to stop the equipment. Only by manually cutting off the power can the situation be resolved. It’s a tragic sight to see equipment continuing to operate without lubrication or cooling! The main shaft bearings, rod bearing shells, and crosshead sliders of the two operating raw material gas compressors (piston type), the two nitrogen-hydrogen gas compressors (piston type), and one circulator (piston type) have all been destroyed. Worked for 5 days and 5 nights with foreign help available!
Due to a small reverse angle in one stage of the compressor, poor lubrication of the crosshead pin led to wear of the small-end bearing shells and damage to the sliding shoes
At Hangyang Turbine Company, it was found that all the bearings on the high and low speed shafts of the oxygen compressor gearbox were damaged, as a result of operating without immediate detection of oil loss.
The worst case I’ve seen was when the bearing of the engine’s turbocharger burned out.
Have seen shafts that are held in place; have also seen bearing shells that hold shafts in place
