Issue of steel plates embedded in walls

I’ve never worked on civil engineering projects, so I’d like to ask everyone: in civil engineering, how is it done to embed steel plates in walls? Should J-shaped rebar be welded to the back of the steel plate before grouting is applied? Is one grouting session more common or two?
Hook nails must be welded, and grouting should be done in one go
