During today’s testing, it was discovered that there was a baffle inside the flange connecting the heat well to the steam trap expansion tank, which caused the liquid level in the steam trap expansion tank to become extremely high and led to significant vibration in the pipelines. The vacuum was broken to remove the plug plate, and thus today’s turbine testing is declared complete. The baffle couldn’t be detected – is there an issue with the inspection procedure, or is the baffle simply too sneaky to be detected? Establish a strict management system for work permits related to the installation and removal of blind flanges; don’t be afraid of the hassle – this time it only resulted in an abnormally high liquid level, but what about next time? Failing to extract what needs to be extracted or to add what needs to be added can lead to serious accidents.
Fortunately, it wasn’t a major issue; the unit was not damaged. In our factory, the turbine bearing surfaces were damaged due to inadequate inspections; it seems that strengthening management through proper systems is crucial!
There is still a lack of responsibility; there should be records of installation and records of removal.
