Welding in an oil tank area caused a building fire. I. Overview of the accident: A shovel manufacturing factory in a certain city violated the safety and fire prevention regulations regarding welding, carrying out welding work near oil tanks, which led to a serious fire accident. Two buildings covering a total area of 20,018 square meters were destroyed, along with 39 pieces of equipment and some products. The direct losses amounted to 79,600 yuan, while the indirect losses were 1,005,700 yuan; the total loss was 1,853,000 yuan. The actions of Guo, the person in charge of the factory’s workshop, and Bai, the welder, violated the Criminal Law. Guo was sentenced to 2 years in prison with a 3-year suspension, while Bai was sentenced to 6 months in prison with a 1-year suspension, in accordance with the law. At 15:20 on April 29, Guo learned that the transmission chain in the workshop’s oiling section was not functioning properly, and he asked the fitters Liu and Liang to inspect it and repair it. Upon inspection by Liu and Liang, it was found that the chain was too long; they suggested cutting off a section of the chain and then welding it back in. After Guo gave his consent, he approached the welding team leader, Zhang, who assigned welder Bai to carry out the welding. Upon arriving at the site, Bai asked Guo, “What can be done with this land?” It’s all oil. ”Liu also said, “This place is dangerous.” ”Guo replied, “I’ve done it before; be a bit careful and get to it quickly.” ”Thus, after Guo Rangliu cut the chain but failed to weld it back together, Bai welded the chain instead. After that, Bai returned to his own class. Guo, Liu, and Liang tried to start the vehicle, only to find that the chain was too tight and was getting stuck in the guide wheel, preventing the vehicle from starting. Liu and Liang proposed to Guo that two sections be cut off from each end of the guide wheel. It was already 4:30 in the afternoon, and the factory notified all its employees to listen to a national radio conference on work safety. Liu and Liang said they couldn’t go on doing it. Guo said, “We have to do it; failing to do so will affect production.” So, Guo called Bai over again. After that, Bai Lai said, “What can be done in this place? It’s all oil.” ”After hearing this, Guo said impatiently, “Why can’t it be done? I’ve done it before. Just be a bit more careful; besides, I’m here too.” ”Liu, Liang and others were also aware of the danger, so they covered the oily areas on the ground with iron plates, and used those same plates to block the side of the oil tank at a distance of 1.4 meters from the cutting point; the iron plates were raised 10 centimeters above the oil tank to prevent sparks from falling in. Liu started cutting first; after cutting for a while, the molten iron that fell ignited the oil stains on the ground, and Liang and others present used foam extinguishers to put out the fire, after which they continued with their work. After Liu cut off the ends of the guide rail at the north and south sides and welded the northern end, he handed the welding torch to Bai, asking him to weld the cut end on the southern side of the guide rail. Bai had been welding for about a minute. Sparks fell into the oil tank, immediately igniting the paint and subsequently setting the factory building on fire, resulting in a major fire accident. II. Analysis of the accident causes 1. Illegal command, forcing workers to carry out work in a risky manner. As the workshop supervisor, Guo ignored safety regulations and gave illegal orders, forcing workers to carry out work in dangerous conditions. Worse still: when workers pointed out the hazards of the work and that welding had caused oil-contaminated ground to catch fire, Guo failed to pay close attention to these issues or take effective preventive measures. On the contrary, he persisted in his own opinions and continued to order the workers to carry out work in violation of regulations; Guo bears direct leadership responsibility for the occurrence of this accident. 2. Implementing incorrect decisions and carrying out work in violation of regulations. Bai was an unqualified official welder; he knew full well that welding near oil tanks was extremely dangerous. Guo’s instructions were clearly wrong, yet Bai took no preventive measures nor challenged his superior’s erroneous decisions, continuing to work in violation of regulations – and he is the direct cause of the accident. III. Measures to prevent similar accidents 1. Attach importance to safety and provide proper guidance. When organizing and directing production activities, workshop supervisors must pay attention to safety, strengthen management, and strictly prohibit giving orders that force workers to carry out dangerous tasks ; It is necessary to carefully listen to correct opinions and suggestions that are beneficial to safe production. Take effective preventive measures to ensure safe production. 2. Adhere to principles and work safely. Production staff must strictly abide by safety production regulations and rules; they should dare to oppose any incorrect command decisions, and it is strictly forbidden to carry out tasks blindly or take risky actions. It’s most cost-effective to learn from others’ accidents as if they were one’s own*.
