An explosion occurred in the steam guide pipe of Boiler No. 1 at Luohé Power Plant. Incident details: At 17:45 on August 22, 1991, Boiler No. 1 was started up; At 20:55 on the 22nd, Turbine No. 1 was started up ; At 06:42 on the 23rd, the grid connection was in operation with a load of 20–30 MW; tests were conducted on the turbine and electrical systems ; At 14:45 on the 23rd, with a load of 60 MW, the first layer of powder feeding was activated ; At 16:00 on the 23rd, the steam guide pipe burst, forcing a shutdown of the boiler. Analysis of the accident cause: The main reason was improper hydrodynamic adjustment of the boiler, which led to uneven water flow in various sections, resulting in overheating and tube rupture of the steam guide pipe.
Accident summary: 1. For tube ruptures caused by prolonged overheating, it is necessary to determine whether the overheating is resulting from thermal imbalances, hydraulic imbalances, or structural defects in the boiler, so that appropriate countermeasures can be taken. 2. The superheater, reheater, steam guide tubes, etc. of the boiler should be equipped with complete wall temperature measurement points, in order to monitor temperature differences between various steam guide tubes and prevent tube rupture due to overheating. During startup, the flue gas temperature in the horizontal flue should be monitored, and the wall temperatures of the superheater and reheater must not exceed the specified values. 3. Regularly perform steam soot blowing in the boiler furnace and flue to eliminate thermal imbalances and prevent localized overheating of the heating surfaces.
