In chemical engineering design projects, in the event of a safety accident caused by design flaws, whose responsibility is greater – that of the process design or that of the piping design? Which has greater responsibility, scheme design or construction drawing design? What are the responsibility percentages for design, verification, review, and project leadership?

Once something goes wrong and it becomes the handler’s responsibility, everything needs to be considered in its entirety. Things like sacrificing a minor element to save the overall situation will happen.
