Maintenance work is a type of high-risk activity; therefore, it is necessary to establish management systems for such work, conduct risk assessments, prepare work plans, implement safety measures, provide safety training, fill out maintenance work forms and obtain approvals, carry out on-site supervision, and also have emergency response plans in place. The workshop where the work is carried out should issue a inspection and maintenance work order, propose process treatment plans and implement measures such as material replacement, blind plugging, assignment of supervisors, and safety protections; the person in charge of that workshop shall review them ； The contractor submits a construction plan, including details on tools, vehicles, lifting equipment, methods for working at heights, materials, the construction process, and the personnel involved, which is then reviewed by the person in charge of the contractor ； The safety and environmental department proposes safety measures and conducts audits and supervision, such as verifying the qualifications of personnel performing special tasks and checking the relevant work permits. Depending on the level of risk assessment, different levels of approval are required. For example, if a control valve fails, it is sufficient to close the valves before and after it, evacuate the area, and then proceed with removal and repair – tasks involving relatively low risks that can be approved by either the local unit or the unit carrying out the work. On the other hand, when a large machine unit needs major repairs, this involves complete replacement of components, isolation, power disconnection, locking and tagging; it may also require hot work, work in confined spaces, temporary electrical wiring, work at heights, lifting operations, as well as plugging and unplugging blind flanges. There is also a risk of cross-interfering activities, which necessitates a joint response from multiple departments and requires approval from higher-level authorities.
